Good afternoon, Mr. Howes. I gained access to this site while conducting a LinkedIn search on sports organizations and groups. I am also interested in the PDF, but I have not even received an e-mail after providing my name and primary e-mail address TWICE in the past twenty minutes. It would be greatly appreciated if you would take the time to directly send the file to my Inbox.

what am I doing wrong? I entered my name and email. And I got an email message from you. But no PDF download link; just a link that brings me back here to, you got it, enter my name and email to get the PDF link ...
